To modify and maintain an existing reinforced concrete slab bridge across Toby Creek (TSF), with work consisting of widening the downstream end of the structure by approximately 20 feet. The existing structure has a waterway opening of approximately 12.0 feet × 7.5 feet at the downstream end, and located on S. R. 0415, Section 301, Segment 0010, Offset 2069, approximately 0.5 mile northwest of the intersection of S. R. 0145 and S. R. 0309 (Kingston, PA Quadrangle N: 15.7 inches; W: 12.1 inches) in Dallas Borough, Luzerne County.

   Permit No. E64-181.  Encroachment. Wayne County Courthouse Annex, Honesdale, PA 18431. To remove the existing super structure and to construct and maintain a single span steel I-beam bridge, having a span of 31.3 feet and an underclearance of approximately 8 feet across Wilcox Creek (HQ, CWF). This project is located along Township Route T-325 (Old Stephens Road), approximately 300 feet west of N. R. 3004 (Sterling, PA Quadrangle N: 21.5 inches; W: 8.5 inches) in Salem and Sterling Townships, Wayne County Philadelphia District, Army Corps of Engineers.

   Permit No. E66-114.  Encroachment. Exeter Township, P. O. Box 88, Falls, PA 18615-9801. To maintain a single span bridge, having a span of approximately 31 feet and average underclearance of 7.3 feet across White Lock Creek (CWF). The project is located along Township Road T336 (Coolbaugh Mountain Road), approximately 0.2 mile south of its intersection with S. R. 0292 (Ransom, PA Quadrangle N: 10.5 inches; W: 15.4 inches) in Falls Township, Wyoming County (Baltimore District, Army Corps of Engineers).

   EA40-002NE.  Encroachment. Pennsylvania Department of Environmental Protection, Bureau of Abandoned Mine Reclamation, Public Square Commons, 2 Public Square, Wilkes-Barre, PA 18711-0790. To place fill in a 4-acre water body in association with an abandoned mine reclamation project (OSM 40(3218)101.1, Pardeesville) under the category 2 blanket waiver guidelines published on December 14, 1996. The purpose of the project is to eliminate dangerous highwalls and hazardous water bodies. The project does not meet the criteria for the category 1 blanket waiver guidelines as published in the Pennsylvania Bulletin on December 14, 1996, (Freeland, PA Quadrangle N: 1.0 inches; W: 10.0 inches) in Butler Township, Luzerne County (Baltimore District, Army Corps of Engineers).

   E41-404.  Encroachment. Muncy Borough, 14 N. Washington St., Muncy, PA 17756. To remove the deposits of gravel for a distance of approximately 5,000-feet and maximum depth of 2.5 feet for a total removal of approximately 9,000-cubic yards from Glade Run from 1,000-feet downstream of the Main St. bridge to 200-feet upstream of Penn Street bridge for the purpose of flood control over a period of 5 years and repair existing, or install new riprap and maintain both as necessary for streambank protection (Muncy, PA Quadrangle N: 14.9 inches; W: 4.8 inches) in Muncy Borough and Muncy Creek Township, Lycoming County. Estimated stream disturbance is more than 250 feet; stream classification is WWF.

   E49-206.  Encroachment. Northumberland County Commissioners, 50 S. Second St., Sunbury, PA 17801. To remove the existing structure and to construct and maintain a single span precast concrete beam bridge with a span of 46 feet and average underclearance of 7.85 feet over Limestone Run located on Ridge Ave., 80 feet north of Race Street (Milton, PA Quadrangle N: 3.0 inches; W: 13.8 inches) in Milton Borough, Northumberland County. Estimated stream disturbance is less than 250 feet, stream classification is WWF.

   E57-077.  Encroachment. Kay M. Wienecke, P. O. Box 85, Hillsgrove, PA 18619. To remove a 300 foot unvegetated gravel bar and to restore, protect with riprap, and maintain the opposite streambank along Elk Creek located next to Elk Creek Road about 1.25 miles north of Route 87 (Hillsgrove, PA Quadrangle N: 17.6 inches; W: 8.9 inches) in Hillsgrove Township, Sullivan County. This stream is designated HQ-CWF and this project will impact 300 feet of stream with no wetland impacts.

   E59-340.  Encroachment. Rex and Ester Willard, R. R. 5, Box 4B, Wellsboro, PA 16901. To remove the existing structure and to construct and maintain a private single span bridge, 20 feet long with an underclearance of 8 feet across Stony Fork Creek located off SR 3007 about 0.8 mile south of SR 3010 (Antrim, PA Quadrangle N: 12.6 inches; W: 14.7 inches) in Delmar Township, Tioga County. Estimated stream disturbance is 30 feet with no wetland impacts, stream classification is CWF.

To remove existing structure and to construct and maintain a 12-foot span arch culvert with 6-foot underclearance in an unnamed tributary to Georges Run (WWF) located on the south side of Lynn Portal Road approximately 800 feet west of intersection of Lynn Portal Road and S. R. 18 (Washington West, PA Quadrangle N: 16.4 inches; W: 5.3 inches) in Canton Township, Washington County.

To relocate and maintain a channel relocation of an unnamed tributary to Byers Run (TSF) for the purpose of installing an access road and self-storage units located north of intersection of Pleasant Valley Road and Chicken Street (Murrysville, PA Quadrangle N: 1.0 inch; W: 6.8 inches) in Penn Township, Westmoreland County.

To construct and maintain a 6-inch PVC sanitary sewer line crossing a deminimus amount of wetlands (PFO) and 10 L. F. of an unnamed tributary to Valley Creek (EV) located approximately 2,600 feet southeast of the intersection of U. S. Route 30 and S. R. 352 (Malvern, PA Quadrangle N: 6.5 inches; W: 7.9 inches) in East Whiteland Township, Chester County.

   E46-771.  Encroachment. Franconia Township, Box 128, 671 Allentown Road, Franconia, PA 18924. To construct and maintain a 45-foot long by 6-foot wide pedestrian/bike path bridge which spans the Skippack Creek (TSF). Also, to install a 130 L. F. boardwalk across the adjacent 100 year floodplain to provide an access to the south side of the bridge. The site is located approximately 2,900 feet west of the intersection of Cowpath Road and Beck Road (Telford, PA Quadrangle N: 7.5 inches; W: 11.5 inches) in Franconia Township, Montgomery County.

   E06-491.  Encroachment. Donald Peters and Robert Nocentino, 425 Mennonite Road, Collegeville, PA 19426. To construct and maintain a 30-inch storm sewer outfall to the Schuylkill River at a point approximately 500 feet upstream of the Reading Railroad (Temple, PA Quadrangle N: 6.0 inches; W: 10.5 inches) in Muhlenberg Township, Berks County.

   E22-366.  Encroachment. Susquehanna Township, Stanley Lawson, 1900 Linglestown Road, Harrisburg, PA 17110. To widen an existing bridge and to construct a bridge having a span of 36 feet and an underclearance of 12.4 feet across the channel of Paxton Creek at points at Progress Avenue and Interstate Drive, (Harrisburg East, PA Quadrangle N: 10.63 inches; W: 14.0 inches) in Susquehanna Township, Dauphin County.

   E36-631.  Encroachment. Southridge Farm Partnership, David Charles, 26 Millersville Road, Lancaster, PA 17603. To locate 250 l. f. of an intermittent tributary to Mill Creek to facilitate stormwater management. To construct and maintain an 8-inch sanitary sewer line, a 4-inch sewer force main and an 8-inch water line across a watercourse. A 42-inch pipe culvert will be constructed in a tributary of Mill Creek under Southrun Drive for access within Southridge Farm development, located along Millport Road (T-541) (Lancaster, PA Quadrangle N: 5.0 inches; W: 1.5 inches) in East and West Lampeter Townships, Lancaster County.

   E67-592.  Encroachment. TDC Galleria West LLC, John Donohue, 513 Central Avenue, Highland Park, IL 60035. To place fill in a deminimus area of wetlands less than or equal to 0.05 acre for the purpose of constructing the Galleria West retail shopping center at a location immediately west of the existing York Galleria Mall (York, PA Quadrangle N: 20.5 inches; W: 7.8 inches) in Springettsbury Township, York County.

DAM SAFETY

To operate and maintain a stormwater detention dam across a tributary to Pine Run (TSF,MF) impacting approximately 0.25 acres of wetlands (PFO) in Buckingham Township, Bucks County. This structure also serves as a roadway crossing in the Buckingham Estates Subdivision.

   The Department of Environmental Protection has taken the following actions on previously received permit applications and requests for plan approval and has issued the following significant orders.

To implement an Erosion and Sedimentation Control Plan for the construction of an addition to an existing school on 65.4 acres in Union Township, Berks County. The project is located between Garey Road and Chestnut Street (Birdsboro, PA Quadrangle N: 1.0 inch; W: 8.4 inches). Drainage will be to a tributary to the Schuylkill River.

   PAS-10-O063.  Individual NPDES. QVC Network Incorporated, 1200 Wilson Drive, West Chester, PA 19380. To implement an Erosion and Sedimentation Control Plan for a QVC warehouse building expansion on 36.16 acres in West Hempfield Township, Lancaster County. The project is located off Stoney Battery Road on to Ivy Drive (Columbia East, PA Quadrangle N: 12.7 inches; W: 7 inches). Drainage will be to a tributary of Swarr Run.

LAND RECYCLING AND ENVIRONMENTAL REMEDIATION

Preamble 3

   The following final reports were submitted to the Department of Environmental Protection under the Land Recycling and Environmental Remediation Standards Act (35 P. S. §§ 6026.101--6026.908).

   Provisions of Chapter 3 of the Land Recycling and Environmental Remediation Standards Act (act) require the Department of Environmental Protection to publish in the Pennsylvania Bulletin a notice of submission of any final reports. A final report is submitted to document cleanup of a release of a regulated substance at a site to one of the act's remediation standards. A final report provides a description of the site investigation to characterize the nature and extent of contaminants in environmental media, the basis for selecting the environmental media of concern, documentation supporting the selection of residential or nonresidential exposure factors, a description of the remediation performed, and summaries of sampling methodology and analytical results which demonstrate that the remediation has attained the cleanup standard selected.

   For further information concerning the final report, please contact the Environmental Cleanup Program in the Department of Environmental Protection Regional Office under which the notice of receipt of a final report appears. If information concerning a final report is required in an alternative form, contact the community relations coordinator at the appropriate Regional Office listed.
